Thank you for your email. I am sorry to hear that you encountered issues or were not able to file a claim successfully on the FTX Digital Claim Portal (https://digitalmarketsclaim.pwc.com/) before the deadline.

We note your intention is to participate in the Bahamas Process.  In order for us to process your request, you must complete the manual (A) Proof of Debt Form and (B) Process Election Form (attached) and return them to us, via this email, before 9:00pm Prevailing Eastern Time on Tuesday, 27 August 2024 .

Failing to send us the required forms by the above deadline will mean we cannot further administer your claim in the Bahamas Process or process your request/query any further.

Further instructions are provided below.  If you have any questions or issues please reply to this email as soon as possible.

Guidance for completing your forms

To participate in the Bahamas Process, you MUST complete both the

(A) Proof of Debt Form and

(B) Process Election Form, by selecting “Option 1: Participate in the Bahamas Process”

(A) Proof of Debt Form

Please complete and sign the attached Proof of Debt form together with the applicable sections. You may refer to the below for the guidance note on how to complete the form:
